Senior Health Services Program

The Senior Health Services (SHS) Program is VAI’s first intensive case management program focused specifically on our senior constituency; it is designed to enhance and strengthen our holistic approach to direct services more generally. VAI’s Senior Health Services Program will connects senior constituents—all of whom are low-income and have limited English proficiency (LEP)—to high-quality primary care services, such as mental health, dental, and vision services offered at our FQHC partner site, Heartland Health Centers and Asian Human Services.  VAI has designed the Senior Health Services Program using the information gleaned from the University of Chicago, the Asian Health Coalition (AHC) and Asian Human Services, as well as our strong understanding of the community needs based on our social service programming. Through the Senior Health Services Program, VAI addresses disparities in health access and literacy in a culturally and linguistically appropriate manners by focusing primarily on health status and health services to:

  • Increase access to effective health care
  • Improve risk reduction and disease prevention
  • Encourage minority participation in the health profession
  • Integrate health literacy and preventative measures
  • Reduce disparities
  • Improve overall quality of care provided
